Weekend Team Nurse (Home-Based Care) RN / LPN

HSWV is growing and re-thinking the way we provide excellent 24 hour care to our patients.

We are hiring home-based nurses to lead a weekend team of care providers for patients in Raleigh, Fayette, Summers and Wyoming Counties. This team will share a flexible day/night shift schedule which alternate weekends to ensure the continuation of care after normal business hours have concluded.

Seeking RN and/or LPN nurses who wish to provide a “Special kind of caring” to patients at end of life.

Nurse candidates must work well in a team environment and be willing to provide hospice care in patient homes. Travel and on-call required. Competitive benefits package and reimbursed mileage. Salary commensurate with experience.

Full-Time LPN (Home-Based Care)

The primary purpose of your job position is to implement and evaluate a patient’s care and patient/family needs through systematic assessment in order to promote comfort and health to patients and their families following the plan of care formulated by the Interdisciplinary Team and Staff RN.

Provides patients and families with skilled nursing care and other support services.

Teaches and counsels patient and family in nursing care and other related issues.

Responsible for reviewing patient chart before visiting patient and speaking directly to the Staff RN and Clinical Care Coordinator for patient update.

Must be available for on-call duties as assigned by the Clinical Care Coordinator.

Responsible for triaging phone calls from nurses, home health aides, patients and physicians.

Cross trains, understands, and complies with expectations for roles for whichhe/she is covering.

Works closely with on-call, admission, case manager and inpatient nurse and is flexible regarding shift changes and coverage.

Travel required with reimbursed mileage. Competitive benefits package and salary commensurate with experience.

Education and Experience:

A graduate of an accredited school of professional nursing who is currently licensed as a LPN in the State of West Virginia. Minimum of one (1) year of professional experience in the delivery of nursing services in Hospice, Home Health or Hospital, preferably two (2) years.

Full-Time Day Shift RN  (Home-Based Care)

Part-Time RN (Home-Based Care)

PRN RN (Home-Based Care)

PRN CNA Day Shift / Night Shift (Bowers Hospice House)

Be a part of an interdisciplinary team of skilled and caring people. Candidates must work well in a team environment and provide “a special kind of caring” to patients in the Bowers Hospice House during a day or night shift on an “as-needed” basis.

Full Time Social Worker (Home-Based Care)

Be a part of an interdisciplinary team of skilled and caring people.

Candidates are required to have a BSW and LSW. Prior experience in the medical field is preferred.

Key functions include but are not limited to: completing psychosocial, spiritual, bereavement and financial assessments; assisting patients with financial and legal matters such as obtaining MPOA, healthcare surrogacy, and preplanning arrangements as necessary; working with families and community resources to solve needs; counseling patients, families, and staff in dealing with feelings about grief, depression, disability, death, dying, or other emotional, mental, environmental and physical limitations.

Salary commensurate with experience. Mileage is reimbursed.  Day shift with weekend availability.

Full-Time Housekeeper (Bowers Hospice House)

The primary purpose of this position is to perform the day-to-day activities of the Housekeeping Department and as may be directed by the supervisor.

Expectations and Responsibilities: As part of an interdisciplinary team of skilled and caring people the Housekeeper must act in accordance with current federal, state and local standards, guidelines, and regulations that govern our organization, to ensure that the Bowers Hospice House is maintained in a clean and sanitary condition at all times.

Education: Must possess a high school diploma or GED.
